Individual LifeStyle Support is dedicated to safeguarding our client’s privacy. We abide by the Privacy Act 1988 and the Australian Privacy Principles 2014. 

Personal information is collected directly from our clients or by third party representatives completing a new client referral form. Our privacy policy is available upon request and will be provided to all new clients.

Individual LifeStyle Support uses consent as our foundation for collection, sharing, and use of information. Consent regarding agreement to our Privacy Policy is gained at the time of first consultation. 

Where a client decides not to provide requested information, we will inform the client of what effects this non-disclosure may have. For example, withholding certain information may limit our ability to provide a quality service.

Clients can withdraw their consent at any time via a written request to Individual LifeStyle Support. If a client withdraws their consent to support, then we will discontinue all services immediately and to the best of our abilities support the client to seek alternative supports if requested or necessary.

Individual LifeStyle Support protects your personal information and we do not share this information with any third party other than for the purpose outlined below:

· For the purposes for which we have advised that we are collecting it, and for related purposes that the client would reasonably expect.

· Where we have the consent of the client to do so. 

· As required by law.

· Under other circumstances where permitted under the Privacy Act.

Any client receiving or having received support services, from Individual LifeStyle Support has the right, under the Privacy Act, to request access to their personal information. If a client requires access to a copy of this information, they must request this via email to: admin@individuallifestylesupport.com.au or by phoning directly on 0434 224 119.

Individual LifeStyle Support records and stores all personal information securely. We regularly review and, where necessary, update our privacy information.

Individual LifeStyle Support makes every effort to ensure that all clients’ personal information is accurate, up to date and correct. Our clients have the right and an obligation to request and keep us informed of any rectification of inaccurate personal information. This can be done by contacting Individual LifeStyle Support directly using the contact details provided.

As Individual LifeStyle Support is committed to protecting the privacy of our clients, we will view unauthorised disclosure of, or access to, personal information by our employees or contractors, as a serious breach of this policy. Appropriate action (which may include disciplinary or legal action) will be taken in such cases.
